Home Safety Assessment

Fall Prevention Training

A series of visits aimed to help prevent falls, before they occur. Build your confidence with fall prevention strategies, strengthening, and addressing environmental risks.

Home Modification Consultations

Collaboration with clients, families, and contractors to suggest functional changes- such as stair railings, bathroom remodels, or threshold ramps- to help you stay at home, safely. Recommendations are tailored to your goals, budget, and home layout. 

What To Expect

Our Intake & Service Process

My goal is to help you or your loved one stay safe, comfortable, and independent at home through personalized home safety assessments, fall prevention strategies, adaptive equipment training, caregiver education, aging-in-place planning, and home modification guidance.

Before your first visit, we will complete a brief phone consultation to review your needs, medical background, home concerns, and goals for services. This helps ensure that the services you request are appropriate and that we can meet your needs safely and effectively. We will also schedule your in-home assessment, where we will complete intake forms and physical consent to services. 

Initial Intake Call

During your in-home evaluation, we will review:

  • Consent for services/ HIPAA

  •  Mobility throughout the home

  •  Bathroom safety

  •  Kitchen and household task safety

  •  Fall risks

  • Durable medical equipment

  •  Lighting, flooring, layout, and accessibility

  • Caregiver needs and communication

  •  Your top safety concerns

This visit typically lasts 60–90 minutes depending on complexity. 

In-Home Visit

A second in-home visit will be completed, providing and reviewing detailed recommendations from the assessment. This report may include: 

  • Recommended adaptive equipment

  • Fall prevention strategies

  •  Environmental safety changes

  •  Caregiver education recommendations

  •  Optional follow-up visit recommendations 

Written Report & Recommendations

Depending on your needs, follow-up services may include:  

  • Equipment setup and training

  • Caregiver training

  • Re-assessment after modifications

  • Annual home-safety check ins
